HEALTHY CONNECTIONS – TRILLIUM OUTREACH COORDINATOR

Job Summary:

The Aging in Place (AIP) Program works to provide enhanced access to a broad range of community services for frail/at risk seniors living in Ottawa Community Housing. The Healthy Connections – Trillium will share the vision of the Aging in Place Program (AIP) and support the mission. He/she will work to enhance the dignity of clients and to foster a safe, supportive and positive environment for them.

Client Services

  • Becomes known to frail/at risk seniors in the buildings
  • Identifies isolated, frail/at risk seniors who need support
  • Facilitates access, refers and integrates tenants to community support services, building activities or other community services for frail/at risk seniors.
  • Plans, develops and implements HC-Trillium programs, special events and services
  • Consults with clients/tenants, caregivers, referring sources, as required
  • Meets with volunteers, clients/tenants, tenants association, paid workers, independent contractors, etc. to discuss program and services
  • Under the direction of the CSS Agency, acts as a resource to support employees, volunteers and contract workers

Public Relations

  • Communicates regularly with all key players (Ottawa Community Housing Worker(s), Tenant’s Association(s), Ottawa Public Health, Community Health Centres, other community partners to enhance the health and social well-being of seniors in identified buildings
  • Meets regularly with key players to problem solve, refers and draws on resources in order to meet the support needs of seniors
  • Works with the Tenants Association to implement programs and group activities in response to identified support needs as appropriate to HCHC Program

Administration

  • Collects and maintains program records, client files and statistical data, inventory control, volunteers, and paid workers/contractors
  • Maintains all appropriate client, data and statistical information and ensures adherence to Program Policies and client confidentiality requirements
  • Attends and contributes to monthly team meetings
  • Contributes to the establishment of policies and procedures related to the program and service area
  • Responsible for security and safety of all office equipment and resources, including financial resources

Qualifications

Education & Experience:

  • Degree/Diploma in one of the following areas: Social Services, Volunteer Management, Gerontology or Nursing
  • 1-2 years working with older adults or persons with a physical disability
  • Program development and facilitation
  • Client assessment and intervention
  • Crisis management
  • Money management and receipt reconciliation
  • Employee supervision and scheduling

Operational Requirements:

  • Clear Criminal Record Check
  • C.P.R and First Aid Certificate
  • Bilingualism (English & French)
  • Access to a vehicle is a requirement.
