The Supportive Care Nurse visits in the home, completes assessments,  ensures that appropriate care plans are developed and implemented as per accreditation requirements. He/she is responsible for the coordination of the of caregiver services, provides support and education to volunteers; represents the hospice to health care agencies and the public. The Supportive Care Coordinator provides public and professional education to groups re hospice role and palliative care.

DUTIES AND RESPONSIBILITIES
Direct Client Service

  • Interprets the role of hospice to client/family and community
  • Ensures that prompt assessments are accomplished, recorded and communicated
  • Liaises with hospice volunteers and staff, family, physician, hospitals community agencies

as necessary to ensure appropriate coordinated services are in place

  • Provides regular follow-up to client/family/volunteer/staff as per accreditation requirements
  • Advocates for client/family with health care professionals and community agencies
  • Assists with and informs the hospice team of transitions to and from hospital as required
  • Arranges for family/volunteer/staff conferences as requested
  • Provides timely information to volunteers and staff on client/family needs

Program Development and Evaluation

  • Works with Executive Director and Board Client Service Committee to evaluate and further develop programs and services
  • In concert with hospice volunteers and staff, identifies community needs and

plans community information and support activities

  • Works with hospital(s) and other community agencies to optimize resources
  • Fosters a team approach to ensure optimal, holistic care for client/family/volunteers

Education

  • Works with Coordinator of Volunteers to orient new volunteers and provides on-going educational opportunities
  • Provides formal and informal educational services to community agencies, institutions, churches and the general public
  • Contributes to relevant educational materials for clients, caregivers and volunteers.            

QUALIFICATIONS

  • Registered nurse with current Ontario registration with 3-5 years community experience
  • Relevant hospice/palliative care experience preferred
  • Effective written and verbal communication skills and experience with group facilitation
  • Strong interpersonal skills to support team building. Ability to foster effective collaboration among members of the Evergreen team.
  • Ability to support communication and effective working practices among members of the interdisciplinary teams supporting Evergreen’s clients.
  • Ability to model effective responses to conflict and crisis situations. Able to act effectively and hopefully in situations of sadness.
  • Valid drivers license and a vehicle required; computer literate – Micosoft office suite and willing to learn hospice specific client database and online technology
  • Strong self-motivation balanced with the ability and desire to achieve team goals
  • Able to work in a fast-paced environment and to adapt to change
  • Demonstrates enthusiasm and commitment to the goals and objectives of Evergreen Hospice.
  • Ability to recognize, validate and support diversity in our approach to client and our community
