We want YOU to bring your expertise to our team as we continue to develop solutions to meet the needs of our local community. If you have initiative, are resourceful, engage easily in teamwork, and most importantly, want to make a difference in healthcare, we want to hear from you!
We are looking for people who want to help patients who are not getting the medical help and social support they need.
The Mental Health Care Navigator position works with our targeted patients with mental health issues and potentially also chronic diseases (e.g. diabetes, kidney disease, heart disease, lung disease, etc.) to improve access to health care and other resources available in their communities. This role requires understanding of their own job boundaries and works with patients, communities, physicians, and the health care team. Patients have multiple challenges and barriers to chronic disease care and the Navigator connects these patients with the broader healthcare system in Toronto. This position is suitable for a non-regulated mental health care worker. The successful candidate must have at least 3 years of experience.
