Advance Practice Clinical Nurse Educator (APCNE)

Safehaven Project for Community Living (Safehaven) is a registered charity that provides residential and respite care to children, youths and adults with medical complexities and developmental disabilities.

Safehaven is looking for an experienced Registered Nurse to fill a key role of an Advance Practice Clinical Nurse Educator that works to align with our mission: To provide high quality, family centred and community-based residential and respite care for individuals with complex care needs.

The APCNE will provide clinical supervision, consultation and direct support (as required) within a community clinical setting. The Advance Practice Clinical Nurse Educator is a member of Safehaven’s leadership team with core competency focus on education for staff, professional development, and collaboration with external partners in the community. Reporting to the Director, Programs and Professional Practice, the APN provides leadership and development of best practice guidelines, policies and procedures, some aspect of research and clinical overview of client population

JOB DUTIES:

  • Observes nursing and related practices for conformance with standards and regulations, prepares reports of findings and recommends program;
  • Provides information and guidance in the development of policies and standards of nursing practice for new programs;
  • Adapts nursing procedures and practices to meet the changing needs of nursing and health care programs;
  • Demonstrates ability to build partnerships and collaborative relationships;
  • Demonstrates the ability to prepare educational material and present to diverse audiences;
  • Demonstrate initiative and creativity in clinical practices;
  • Identifies program needs, and projects the impact of changes on programs and services;
  • Consults with and provides training to staff through the provision of visits, workshops, and information technology;
  • Demonstrates the ability to coach mentor and motivate staff to create an environment conducive to learning and team building;
  • Provides instruction and training to staff by demonstrating nursing techniques or explaining principles and theory;
  • Ensures provision of family-centered nursing care to individuals with medical complexities, chronic conditions and life-sustaining technology in a home like setting;
  • Assesses children's needs, collect and compile data, identify problems, develops, communicates, implements and evaluates care plans;
  • Works closely with interdisciplinary team members internally and externally in the development, implementation and monitoring of a transitional care and best practices;
  • Monitors, reviews and makes recommendation on the organization’s health care policies;
  • Consults and collaborates with community partners in ongoing program growth and development;
  • Oversees client health care records including information regarding clients and their families, nursing observations, telephone communications, interventions, teaching and care evaluations;
  • Delegates appropriate tasks to registered staff, Registered Practical Nurses (RPN) and Residential Support Workers (RSW);
  • Provides health care support to staff in other program areas within the organization as required;
  • Follows all policies and procedures, established by the organization as they pertain to the operation of a residence, and the health, safety and welfare of clients in care, and as they pertain to employee conduct and performance;

Education and Experience:

  • University Degree in Nursing (BScN); Masters preferred;
  • Registered Nurse with current certificate of competence from the College of Nurses of Ontario;
  • Minimum three (3) years of recent and relevant experience;
  • Previous Nursing experience in pediatrics; or working with chronic conditions, medical complexities and technologies;
  • Completion of a Basic Cardiac Life Support (B.C.L.S)program, Level II: Basic Rescuer certification;
  • Theoretical and practical experience in adult education including educational program planning, development and evaluation;
  • Demonstrates excellence in teaching, problem-solving and critical analysis
  • Demonstrate ability to communicate effectively with coworkers, interdisciplinary team members, clients and their families;
  • Understands and delivers to meet the goals and strategic objectives of the organization and the ability to quickly adapt to evolving workplace environment;
  • Strong computer skills, with excellent oral and written communication skills;
  • Successful applicants must provide a recent satisfactory vulnerable sector criminal reference check and medical clearance certificate;
  • Must be able to work a flexible schedule – including occasional evening and weekend work;
  • Travel is required for this position, the successful candidate must have access to reliable transportation;
  • Ability to perform delegated tasks independently or with minimal supervision;
  • Required to respond to a flexible work schedule involving shifts and/or emergency staffing needs in the location and/or emergency needs of the clients in care;
