The Clarence-Rockland Family Health Team (CRFHT) is located in a state-of-the-art, fully computerized medical centre 30 minutes east of Ottawa. We are a true “team” where everybody – physicians, nurses and allied health care professionals – works together to provide comprehensive, accessible and coordinated primary health care to the residents of Clarence-Rockland.

We are looking for an enthusiastic, energetic and motivated person to fulfil the role of Nurse Practitioner-Primary Health Care for a 1 year full-time contract position with the possibility of becoming permanent within our dynamic and supportive interdisciplinary team.

The Nurse Practitioner – Primary Health Care (NP-PHC) will function as a member of the primary care team in the provision of primary health care services. The NP provides services to patients who include the individual throughout their life span, the family, and the community, with a strong emphasis on health promotion, patient education and disease prevention. The NP provides primary care functions and participates in the development, implementation, monitoring and evaluation of the CRFHT programs and services.


  • Nurse Practitioner with a Specialty Certificate in Primary Health Care, NP-PH
  • Baccalaureate of Science in Nursing required, Master of Science in Nursing preferred
  • Current registration with the College of Nurses of Ontario as an RN(EC) required
  • Current Membership with RNAO or NPAO and evidence of liability insurance
  • Current Ontario Driver’s License
  • Valid CPR certification
  • Three to five years nursing experience preferably in a community and/or primary health care setting
  • Excellent knowledge of the health problems that can arise in a family medicine practice for all clients from birth to old age
  • Excellent knowledge of primary and secondary prevention of the most common chronic diseases
  • Excellent clinical judgment and good initiative
  • Superior interpersonal, communication and organizational skills
  • Demonstrated aptitude in interacting with health professionals and within an interdisciplinary team
  • Experience developing, implementing, monitoring and evaluating programs.
  • Experience in individual and/or group teaching
  • Prior experience with an electronic medical record
  • Proficiency in the use of the computer hardware and software, particularly in Microsoft Word, Excel, Outlook
  • Able to work evenings and weekends
  • Able to communicate fluently, verbally and in writing, in both French and English
