Position Summary

The Family Support Counsellor provides individual and family support to youth experiencing family breakdown and are at-risk and or experiencing homelessness.  The Family Support Counsellor will work with youth and their families to explore the difficult issues affecting their relationships. Applying a solution-focused approach, the goal of our program is to help youth and their families develop positive relationships, increase communication and re-establish healthy and supportive relationships with family. In an effort to prevent homelessness and family breakdown the Family Support Counsellor will utilize diversion strategies to establish a plan to keep youth out of the shelter system.  This program provides supports in home, school, Mississauga and Brampton Youth Shelters and community settings. Our Place Peel currently has 1 Fulltime, 14 month contract, beginning June 2019 – July 2020. This position requires flexibility to work evenings and weekends.

 
RESPONSIBILITIES

Prevention, Intervention and Counselling

  • As an employee of Our Place Peel, abides by all the policies and procedures of the agency.  Has a knowledge of and ability to carry out all safety requirements and emergency procedures.
  • Apply client-centered approach when working with clients and their families to achieve positive outcomes.
  • Mediate conversation between youth and family to help open up lines of communication.
  • Actively network within the community to promote the program and services by conducting presentations, attending information sessions and sitting on committees.
  • Provide guidance and counseling to youth and families to prevent homelessness and family breakdown through diversion.
  • Participates in regular supervision, clinical care reviews and team meetings to ensure effective delivery of Family Support services and continuity of care.
  • Collaborates with other agencies that work in the areas of youth homelessness and family support, by participating in meetings and community events.
  • Utilize outcome assessment tools to provide accurate case planning and outcomes.
  • Empowers youth through strength based practices.
  • Supports youth and family with referrals to appropriate professional services.

Administrative and Other Related Activities

  • Compiles statistical information and prepares monthly reports as required for program evaluation and development.
  • Maintain detailed and appropriate documentation for care planning and record keeping, progress notes, and care management plans.
  • Participates in weekly clinical team supervision, as well as monthly individual supervision.
  • Participates on committees with other agencies to enhance and ensure coordinated services
  • Helps facilitate weekly team meetings to review the youth care plans and provides care updates.
  • Completes all monthly reporting, and administrative reports on time. (i.e. mileage, program expenses)
  • Carries out other related duties as deemed necessary by Management

 

Qualifications: 

  • BSW, MSW or equivalent degree in an accredited program.
  • 3 years’ experience supporting youth at risk.
  • 3 to 5 years’ counselling experience in evidence-based brief therapy modalities (Solution-focused, Motivational Interviewing, Narrative Therapy, CBT, DBT etc.)
  • Strong knowledge of family systems theory
  • Experience developing and implementing group work
  • Understanding of the Housing First For Youth model, as well as prevention and diversion strategies
  • Understanding of the barriers faced by youth both within their identified families as well as in the community.
  • Supports and counsels through trauma informed care
  • Works from an anti-oppressive framework.
  • Strong self-motivation
  • Demonstrated and proven ability to engage youth facing complex barriers, as well those who may have been victimized (i.e. sexual exploitation, abuse, neglect)Strong presentation skills
  • Strong interpersonal communication, advocacy, problem solving and negotiation skills.
  • Strong organizational, effective time management and planning skills.
  • Excellent writing and documentation skills including the ability to complete all required documentation.
  • Valid CPI/ UMAB certification.
  • Valid First Aid/ CPR certification.
  • Knowledge of OW, PHIPP, ODSP, CAS and other forms of financial aid to obtain housing or prevent housing loss.
  • A valid Ontario Driver’s License, access to a vehicle and $2,000,000 (minimum) liability car insurance coverage is mandatory.
  • Proficient with computers, MS Office products, database entry and compiling statistics.
  • Satisfactory Police Records Check and Vulnerable position that meets the standards of the agencies policy.
  • Must be able to work flexible hours including weekends and evenings
