Family Support Worker

Permanent Part-time (28 hours per week)

Senior Manager, Clinical Services





This position is responsible for providing case management and individual, family and group support, supportive counselling, advocacy and education to families of adolescents with developmental disabilities, who are between the ages of 12 and 21 and who are living with their families.


Key Responsibilities:


·         Supports families in accessing and advocating for services in the community and navigating the system (e.g. applying for funding; exploring suitable program options in the community).      

·         Networks and provides consultation with community partners to support and facilitate the coordination of services for families.  Collaborates and consults with community supporters/services when there are crises. Assists the Lead Agency in implementing a strength-based, family centered, coordinated service plan.

·         Completes needs assessment and on-going service reviews; collaboratively develops service plans with families to achieve their goals.

·         Prepares documents and other resources for clients and families: takes clinical notes during FSW appointments, enters all direct and indirect services into CYSIS, and prepares assessments and reports.

·         Provides supportive counselling to families in dealing with stresses of coping with child / children with developmental disabilities.                                    

·         Educates and brings awareness to families regarding resources in the community by developing presentations and education modules for families and community supporters (i.e. Parents’ support group).             

·         Actively contributes to the Agency’s Strategic Plan by participating in initiatives and committees to support the overall Plan.

·         Takes all reasonable and necessary precautions to protect his or her own health and safety and that of co-workers by complying and demonstrating knowledge of the policies, procedures and safe practices established by YouthLink.







·         Undergraduate Degree in Social Work, Psychology or Degree / Diploma in Child & Youth Worker.

·         Specialized training in diversity or anti-oppression. Mental health training in relation to people with developmental disabilities.



·         Two (2) years’ experience working directly with individuals with developmental disabilities. 

·         Previous case management experience.  Familiarity with life and social skills development support for individuals with developmental disabilities

·         Previous formal or informal counselling experience.




·         Strong interpersonal and communication skills.

·         Ability to stay current on the changes in Ministry guidelines regarding supports and programs for individuals with developmental disabilities. 



·         The position requires evening hours of two (2) evenings per week. GTA travel required, access to a vehicle an asset.




28 hours per week



$ 45,613 – 51,675 (to be pro-rated on hours worked)
