The Clarence-Rockland Family Health Team (CRFHT) is located in a state-of-the-art, fully computerized medical centre 30 minutes east of Ottawa. We are a true “team” where everybody – physicians, nurses and allied health care professionals – works together to provide comprehensive, accessible and coordinated primary health care to the residents of Clarence-Rockland.

We are looking for an enthusiastic, energetic and motivated person to fulfill a full-time Social Worker positions within our dynamic and supportive interdisciplinary team.

We are looking for one candidate who has experience evaluating social needs of patients suffering from memory impairment (dementia, Alzheimer’s) and/or who has experience in children and youth counseling/therapy.

Requirements:

  • Graduate degree in Social Work from a recognized university (Master’s prepared Social Worker will be given preference)
  • Registration with the Ontario College of Social Workers and Social Service Workers of Ontario
  • Additional supervised clinical training
  • Able to communicate fluently, verbally and in writing, in both French and English
  • Three to five years counseling/therapy experience, preferably in a community or primary care setting
  • Proficient in assessment, psychotherapy and psychological counseling techniques for working with diverse populations
  • Able to work both autonomously and collaboratively within an interdisciplinary team
  • Experience developing, implementing, monitoring and evaluating programs
  • Strong knowledge of community resources
  • Good understanding of the legal framework as it applies to social work
  • Demonstrated experience in individual and group teaching
  • Prior experience with an electronic medical record
  • Superior social, interpersonal, communication and organizational skills
  • Able to work evenings and weekends as required

Desired experience:

  • Knowledge of chronic disease management strategies
  • Skilled in, or knowledge of, short-term interventions and/or cognitive-behavioral therapy
  • Experience in helping patients navigate the health system and community resources
  • Experience with a variety of ethical issues commonly faced in primary care
