The Canadian Mental Health Association Peel Dufferin Branch (CMHA Peel Dufferin) is pleased to announce that we are currently recruiting for BSO Crisis Mental Health Nurse.

 Salary band is $67,700 - $77,700 per year

 Summary of Service

The Behavioural Supports Ontario (BSO) initiative was created to enhance health care services for older adults in Ontario with complex and responsive behaviours associated with dementia, mental health, substance use and/or other neurological conditions.

The BSO Crisis Mental Health Nurse will provide intensive crisis and transition support for older adults with cognitive impairments due to mental health problems, addictions, dementia or other neurological conditions with a focus on addressing responsive behaviours. In addition to supporting crisis situations, the BSO Crisis Mental Health Nurse will conduct episodic visits to the client in their preferred setting with a focus on risk reduction and support.

Additionally, the BSO Crisis Mental Health Nurse will work to integrate, coordinate and collaborate with families as well as existing support services in Caledon and Dufferin to development of a care plan.

As a mobile worker, the BSO Crisis Mental Health Nurse is required to work in multiple settings across the region to support the client in their preferred place of residence.

The BSO Crisis Mental Health Nurse will work in collaboration with 24.7 Crisis Support Peel team, including the various Police forces in the region. This includes a systemic approach for the assessment of adults in crisis and those who are experiencing responsive behaviours to stabilize and manage the current crisis situation. The Nurse conducts risk and safety assessments, gives support and accesses support from many sources in the community.

This is a full time permanent position with a schedule that includes some evenings and weekends. The position is based in Caledon and Dufferin County and reports to the Manager, Dufferin Bolton Caledon Care Collaborative.

 

Key Responsibilities:

  1. Provides intensive crisis and transition support for older adults with cognitive impairments due to mental health problems, addictions, dementia or other neurological conditions with a focus on addressing responsive behaviours.
  2. Conduct episodic visits to the client in their preferred setting with a focus on risk reduction and support.
  3. Support, coordinate and collaborate with families as well as existing support services in Caledon and Dufferin.
  4. Conducts risk and safety assessments, gives support and accesses support from many sources in the community
  5. Utilize Coordinated Care Plans to improve health outcomes for individuals supported
  6. Complete a mental health assessment for clients in crisis, utilizing a variety of assessment tools.
  7. Support the caregivers by providing education and advocacy.
  8. Form positive and strong partnerships with hospitals, long term care facilities, law enforcement agencies, and mental health and social service agencies to build an effective delivery of crisis service in the community. 
  9. Work in collaboration with 24.7 Crisis Support Peel Dufferin team, including Police Officers, using a systemic approach for the assessment of adults in crisis and those who are experiencing responsive behaviours to stabilize and manage the current crisis situation.
  10. Provide interdisciplinary team support across the Central West BSO Networks.
  11. Work with BSO staff to promote cross sector transitions and person centered care for clients and family caregivers
  12. Attend and actively participate in team meetings and other client-related meetings and program meetings as required.
  13. Maintain appropriate record-keeping/documentation and client files, statistical and outcome measurement tools as required by CMHA/Peel Dufferin policies and procedures.
  14. Submit BSO MOHLTC Activity Tracking data quarterly or at the request of the Director for the Central West LHIN BSO Network
  15. Participate on agency and community committees and workgroups as requested.
  16. Work effectively and collaboratively with program teams, internal and external to CMHA/Peel Dufferin.
  17. Other related duties as assigned.

 

Skills & Qualifications:

  1. Bachelor’s Degree in Nursing B.Sc (N) is required.
  2. Must be registered with Ontario College of Nurses.
  3. Minimum 5 years of experience working in acute mental health sector, preferably in the community.
  4. Demonstrated and extensive knowledge and skill working with elderly individuals with various types or dementia delirium, mental health, addictions, other neurological conditions and those who experience an acute behavioural change.
  5. Knowledge of Community Support Services, Primary Care and Mental Health systems and related legislation.
  6. Knowledge of consent and capacity issues and legislation regarding this population  ( e.g. role of Public Guardian and Trustee, SDM act  health care consent act, Mental Health Act).
  7. Knowledge and application of the P.I.E.C.E.S. (copyright) framework and templates
  8. Utilization SBAR tool in communication with physicians and clinical partners.
  9. Demonstrated experience with risk assessments, suicide prevention, crisis intervention techniques and theory-based practice in working with the seriously mentally ill.
  10. Ability to work with clients of diverse cultural and social backgrounds is necessary.
  11. Ability to work collaboratively and in partnership with a multidisciplinary team and other agencies in the community, including the ability to work with police, court system, inpatient and outpatient mental health systems.
  12. Able to function well under pressure and in a fluid environment.
  13. Bilingualism (French/English) and/or proficiency in a second language would be an asset.
  14. A valid Canadian driver’s license, access to a reliable vehicle and relevant insurance.
  15. A satisfactory Vulnerable Sector Screening (Police Check) report and extensive background check is required.
  16. Current CPR/First Aid certification.
  17. Ability to work in some evenings and some weekends.
