Relief Outreach Caseworker

This is a relief Union Bargaining Unit position in the Case Management and ACTT Programs.


Purpose of the Position:        

To provide moderate to intense Case Management and ACTT services for consumers of mental health services through the use of a flexible, comprehensive, and individualized model of support; and to implement the goals and objectives of Summit Housing & Outreach Programs.

Key Requirements:

  • Degree in a Mental Health discipline or Social Work or Community College Diploma coupled with case management experience will be considered.
  • Registered health care professional in good standing preferred.
  • Case-management & supportive housing training and experience. ACTT experience an asset.
  • Experience with individuals with serious and persistent mental illness.
  • Excellent assessment, crisis intervention, problem solving and organizational skills.
  • Sound knowledge of the principles of Psychiatric Rehabilitation and Recovery.
  • Ability to work both independently and cooperatively within a community based service provider’s team.
  • Excellent communication and interpersonal skills.
  • Demonstrated ability to be flexible in work assignments.
  • Sound knowledge and understanding of systems issues, legislation, Mental Health Reform, and patient entitlements for the seriously mentally ill.
  • Basic understanding of psychotropic medications.
  • Knowledge of housing, social, financial, vocational and other community resources.

Expertise with support and guidance, when required, to individuals to facilitate learning the activities of daily living, e.g. budgeting, cooking, cleaning, shopping, personal hygiene, social and recreational activities.

  • Skill with providing a safe and efficient operation of the Summit Housing & Outreach Programs’ residences, and ensure that a satisfactory standard of nutrition and cleanliness is maintained.
  • Ability to recognize, assess, respond and/or diffuse crisis situations.
  • Valid driver’s license and access to a reliable vehicle, adequate insurance coverage is essential.
  • Willingness to work shifts as assigned, including weekends and statutory holidays.



Day availability is essential, evenings, weekends and holidays may also be requested

250 hours probationary period: $19.66 per hour plus 1% lieu of benefits

Job Rate following the probationary period: $21.31.


Commencement Date:         

As soon as possible.
