York Region and South Simcoe



The Occupational Therapist (OT) is part of an interdisciplinary team that provides consultation, direct care, capacity enhancement and support to older adults (and their partners in care), living in the community; within the geographical boundaries of York Region and South Simcoe. The OT is expected to ensure the safety and security of the client(s) environment, to practice in accordance with the College of Occupational Therapists of Ontario standards, and the policies and procedures as defined by LOFT Community Services. The OT will be responsible for working with individuals from diverse backgrounds, contributing to Integrated Psychogeriatric Outreach Program development, and providing support to the physical, cognitive, mental health and social/recreational needs of older adults and seniors in a way that enhances their dignity and personal empowerment. The OT is directly responsible for developing and articulating strategies for the management of recovery, psychosocial rehabilitation and coordination of care using information collected through assessments and collaboration with the integrated care team. Additionally, this role will liaise with community agencies, other practitioners and key partners, including caregivers, to coordinate care for clients.



  • A Master’s degree in Occupational Therapy from an accredited institution;
  • Must be registered and in good standing with the College of Occupational Therapists of Ontario;
  • Up to three (3) years of experience in the provision of clinical psychogeriatric services.
  • Demonstrated strong ability to deliver services within OT professional scope of practice; 
  • An understanding of recovery and psychosocial rehabilitation principles and the therapeutic relationship;
  • Well-developed problem solving abilities and interpersonal skills;
  • Self-directed, results oriented and capable of working independently as well as in a team environment;
  • Good oral and written communication skills and computer literacy;
  • Excellent assessment skills, including demonstrated knowledge of crisis intervention skills;
  • Demonstrated ability to provide individual/group health teaching to clients, caregivers and other care providers;
  • Ability to collaborate effectively with other members of inter-professional care teams, clients, family members and community team members;
  • Demonstrates awareness and understanding in working with individuals from different cultures;
  • Demonstrates initiative and creativity in the workplace;
  • A positive attendance record;
  • Ability to collaborate effectively with other members of inter-professional care teams, clients, family members and community team members;
  • Criminal Records Check and Vulnerable Sector Screening;
  • Valid Driver’s License and Vehicle required as services will be provided across York Region and South Simcoe
