The Social Worker, as part of the interdisciplinary team, assists in the primary care functions and participates in the development, implementation, monitoring and evaluation of programs and services. In addition, they ensure that high quality patient care is delivered, consistent with the philosophy and standards of the Georgina NPLC and the Ontario College of Social Workers and Social Service Workers.
The Social Worker is responsible for providing comprehensive on site mental health assessments for individuals, families and groups. They will assist clients with recovery from emotional distress; facilitate improvements in the social, financial and living conditions that affect a client’s health and wellbeing; provide abuse prevention programs; provide mental health treatment and follow-up as determined in consultation with the patient and the primary care provider (NP-PHC). The Social Worker will also liaise with the broader mental health system (such as CCAC, ACT Teams, Central LHIN, Detoxification Centres, Community Treatment centres and CAMH) to facilitate continuity of care for patients requiring more intensive intervention.
