Are you living with a chronic health condition, care for someone who does, or are passionate about helping others? If you are comfortable speaking in front of small groups and interested in helping others to build their skills to live well with a chronic health condition, we have a Peer Leader Training opportunity for you!
We are seeking long-term volunteers within the Central West Region (Brampton, Malton, Dufferin County, Caledon, Woodbridge, Rexdale) to become Peer Leaders through certification with the Chronic Disease Self-Management Program.
The Chronic Disease Self-Management Program is an evidenced-based workshop series (developed by Stanford University) runs for 2.5 hours, once a week, for 6 consecutive weeks, led by 2 Peer Leaders. It helps participants develop new tools and skills to manage symptoms and feelings that can result from living with chronic health conditions. Workshop topics include: actions plans, problem solving, physical activity, nutrition, relaxation techniques, positive thinking, and working with healthcare providers.
As a trained Leader, you commit to deliver a minimum of one 6-week program each year for the Central West Self-Management Program to remain certified. In appreciation for your time, an honorarium + travel is reimbursed to each Leader upon completion of a full 6-week workshop series.
