Health for All Family Health Team (FHT) is an inter-professional primary care clinic with a mission to provide excellent patient-centred care and invest in the health care practitioners of tomorrow. We offer a wide range of primary care services including acute, chronic and preventative health care. Importantly, the FHT is also the site of the Markham Family Medicine Teaching Unit, a partnership with Markham Stouffville Hospital and the University of Toronto Department of Family and Community Medicine.
Our team of health professionals comprises family physicians, registered nurses, nurse practitioners, a physician assistant, pharmacist, dietitian, case manager and social workers, all working together to provide primary health care for our community. We provide home visits and are skilled in geriatric care, low risk obstetrics, global health and palliative care. We strive to serve our community in a dynamic clinical and educational environment that supports our belief in Health for All.
We are active partners in the new Eastern York Region North Durham OHT with plans to focus first year efforts on patients with mental health challenges and/or addictions and those living with dementia and their caregivers.
