Transitional Aged Youth Counsellor

Reports to: Transitional Aged Youth Program Manager


The Transitional Aged Youth Outreach Program provides intensive case management and counselling support to youth ages 16-24 who have experienced substance using challenges and barriers related to staying connected to relevant support services. The substance involvement has impacted a young person’s ability to manage other life areas and oftentimes, mental health related concerns exist. The program’s design is built on a resiliency and strength based approach and incorporates harm reduction practices.  The program also provides support and education to caregivers (both family and other service providers) associated with these young people. The majority of services occur in the community utilizing an outreach support.

Essential Duties and Responsibilities:

  • Provide specialized substance assessments to transitional aged youth, or to collateral clients, through the use of diagnostic tools and structured interviews (client base is Halton region and west Mississauga). GAIN certification considered an asset 
  • Develop treatment plans in consultation with the client and work closely with client to operationalize the plan
  • Provide individual, family and/or group counselling support
  • Oversee youth group. Experience with DBT and Resiliency models of practice considered an asset.
  • Oversee caregiver group program
  • Provide consultation and education services to community services and/or professionals
  • Maintain accurate and complete client records in accordance with agency guidelines
  • Maintain accurate computer data entry in accordance with agency guidelines
  • Assist in the promotion of ADAPT and participate in program development as required


  • Experience working with 16-24 year old youth in the area of addictions and mental health
  • Experience in individual, family and group work
  • GAIN certification considered an asset 
  • Experience with DBT and Resiliency models of practice considered an asset.
  • Addictions Studies Certification, degree/diploma in Social Service and/or Health related field or equivalent combination of experience and education
  • Strong interpersonal skills, ability to work independently as well as with a team, good organizational and communication skills,
  • Ability to communicate effectively with clients and employees at all levels of the organization
  • Ability to effectively present complex clinical information in writing and verbally
  • Ability to use common office software and ability to adapt to new software and applications
  • Must have valid license and reliable vehicle
  • Must undergo police records check
