Position Overview: Under the strategic direction of the Manager, Health Services, the Case Worker functions interdependently with staff to provide shared, client-centred care. This role will be responsible for supporting internal and external referrals, applications and documents. The Case Worker collaborates with primary and allied care, community services and agencies to facilitate the connection with our clients.

In this role, you would contribute to an integrated team-based model focused on creating impactful care pathways related to social services. You will be pivotal in fostering an inclusive community health setting where equity-deserving and people with physical disabilities experience the highest quality access to care. Summary of Duties
• Support clients with healthcare resources in any context; culturally appropriate

approaches; trauma informed practice; integrating promising and emerging practices


• Client care coordination, including orientation, assessment, referrals to internal and external resources, case management and discharge planning


• Participate actively in Vibrant’s interdisciplinary team, supporting integrated approaches to client services based on social determinants of health


• Participate internally and externally in case conferences as a member of client’s circle of care


• Develop and maintain both informal and formal community networks and partnerships and work effectively with community partners to implement collaborative programs and initiatives, and support capacity building.


• Provide service navigation to clients including ODSP and OW support
• Work with community groups and organization to identify service gaps and needs
• Network, liaise and jointly advocate with organizations and community groups

addressing issues identified by clients and/or community
• Participating in primary health care program and service delivery within the broader

community context in partnerships with other institutions as may be required from time to time

Qualifications/Skills
• Graduate degree in Social Work, Community Services or a related health/human

services field or Registered Social Worker (BSW/MSW) with the Ontario College of Social Workers in good standing


• Minimum 3 years in a community-based setting working with vulnerable populations.
• Knowledge of case management and social work theories and practice
• Demonstrated ability, commitment to and knowledge of the community health care

model including the social determinants of health and harm reduction principles.
• Knowledge of available social services resources within the City of Toronto.
• Proficiency in the use of computers and various software applications, including

experience using Electronic Health Record Systems (EHRs)
• Experience using PS Suite is an asset
• Experience with documentation related to case management and data collection.
• Demonstrated ability to work independently and as part of an interprofessional team
